About the Role
  
You'll provide specialist telephonic nutrition care and care coordination to members through comprehensive person-centred care planning or utilising an existing suite of contracted service providers.

What would you do as our Dental Practice Manager?

  • Identify and engage Members who may benefit from chronic disease management services through outbound contact processes.
  • Conduct telephonic healthcare assessments with Members.
  • Provide quality, safe, appropriate information to members with health/care needs.
  • Person-centred care planning, according to member goals and immediate health needs; referring people with care needs to other service providers as required; regular review to assess outcomes and modify care plans as appropriate.

What are we looking for?

  • Accredited Practising dietitian – current unrestricted registration with Dietitian’s Australia
  • Computer Literacy – competency in MS Office suite
  • 5+ years (full-time equivalent) experience as an Accredited Dietitian.
  • Experience in Health Coaching
