Health and Wellbeing Consultant


The Health and Wellbeing Consultant ( 12 month secondment ) will be responsible in assisting with the implementation of HCFs Health and Wellbeing Strategy.

About the role
In this role, you will be responsible in assisting with the implementation of HCF's Health and Wellbeing Strategy to plan and ensure operational efficiency. You will also manage and partner with key internal stakeholders to assist them with their health and wellbeing needs.


Key responsibilities include:

  • Maintain and liaise with third party suppliers to deliver chronic disease management programs according to the Health and Wellbeing strategy as well as contractual obligations
  • Recommend enhancements/expansion/rationalisation to current programs
  • Assist with measuring outcomes of Chronic Disease Management Programs
  • Liaise with Marketing in delivering comprehensible and accurate messaging to members about health and wellbeing
  • Ensure timely resolution of complaints related to health and wellbeing programs
  • Implement Trajectory of Care project activities/processes
  • Support Frontline staff with member enquiries regarding current disease management programs
